The Markers Men Should Start Tracking at 40

Not because something goes wrong at forty, but because it is roughly when the conditions that decide the next several decades become measurable — and still modifiable.

Men see physicians less often than women across most age groups and are more likely to present late with conditions that were detectable earlier.

Forty is a reasonable point to change that pattern. Not because something goes wrong at forty, but because it’s roughly when the slow-developing conditions that determine the next several decades become measurable — and still modifiable.

Cardiovascular

Blood pressure. Measured properly: seated, rested five minutes, arm supported at heart level, appropriately sized cuff, and confirmed on more than one occasion. Home monitoring across several days gives a better picture than one clinic reading, which is subject to white-coat elevation.

Standard lipid panel. Total cholesterol, LDL, HDL, triglycerides.

ApoB. Worth asking about. It counts the number of atherogenic particles rather than the cholesterol they carry, and in people whose particle number and cholesterol content diverge — common with insulin resistance and elevated triglycerides — LDL cholesterol can understate risk. Increasingly discussed in cardiology, though not yet standard in routine screening everywhere.

Lipoprotein(a). Largely genetically determined, associated with cardiovascular risk, and not included in standard panels. Because it’s genetic, it generally needs measuring only once in a lifetime. If elevated, it changes how aggressively other risk factors warrant management.

Coronary artery calcium score. A low-radiation CT quantifying calcified plaque in the coronary arteries. Provides direct evidence of existing disease rather than risk estimation. Whether and when it’s appropriate depends on your overall risk profile — a conversation to have rather than a default.

Metabolic

Fasting glucose and fasting insulin, together. Glucose alone moves late. Insulin reveals the compensation that precedes it.

Hemoglobin A1c. Average glucose over roughly three months.

Waist circumference. A tape measure captures visceral adiposity better than weight or BMI.

Liver enzymes, with attention to fatty liver. Non-alcoholic fatty liver disease is common, closely tied to insulin resistance, and often silent. Mildly elevated ALT deserves follow-up rather than a shrug.

Body composition and function

Muscle mass and body fat, ideally by DEXA. Establishes a baseline for the trajectory that most determines functional independence later.

Grip strength. Cheap to measure, correlates with whole-body strength, and associated with a range of outcomes in cohort studies.

VO₂ max, measured or estimated. One of the more robust single predictors of all-cause mortality.

Bone density, if risk factors are present. Men develop osteoporosis, are screened less, and have worse outcomes after hip fracture.

Hormonal

Testosterone, if symptoms warrant it — morning sample, confirmed, with SHBG and LH for interpretation. Screening asymptomatic men isn’t generally recommended.

Thyroid function. TSH as a starting point, with further testing if it’s abnormal or symptoms suggest it.

Other

Complete blood count and comprehensive metabolic panel. Standard, broad, occasionally informative.

Vitamin D. Deficiency is common, particularly at higher latitudes and in people with limited sun exposure.

Colorectal cancer screening. Guidelines in the United States now generally recommend beginning at 45 for average-risk adults, earlier with family history. This one has clear evidence behind it and is frequently deferred.

Prostate discussion. PSA screening involves real tradeoffs between detecting significant cancers and overdiagnosing indolent ones. Current guidance emphasizes shared decision-making rather than universal screening. Worth an actual conversation about your risk factors, not an automatic yes or no.

Skin check, particularly with significant sun exposure history.

Sleep apnea evaluation, if there’s snoring with witnessed pauses, unrefreshing sleep, or persistent daytime sleepiness. Common in men, frequently undiagnosed, and it drives blood pressure, glucose regulation, and testosterone simultaneously.

How to use this

Two things make this list worth something.

Establish a baseline. A single set of results tells you where you sit relative to a population. A series tells you your direction — which is the more useful information, and it requires starting somewhere.

Read them together. Individual values near the edge of normal are easy to dismiss one at a time. Several related markers drifting in the same direction mean something the line-by-line format obscures.

Not everything here is appropriate for everyone, and some items involve genuine tradeoffs rather than obvious answers. The list is a starting point for a conversation with a clinician who knows your history — not a menu to order from.
