A single total testosterone result is the most common way men encounter this topic — one value, compared to a reference range, producing either reassurance or concern.
That single number is a genuinely poor basis for either conclusion, for several independent reasons.
The number moves constantly
Testosterone follows a daily rhythm. Levels are typically highest in the morning and decline through the day. A sample drawn at 8 a.m. and one drawn at 4 p.m. from the same person can differ substantially. This is why guidelines specify morning collection.
It varies day to day. Repeated measurements in the same individual across days show meaningful fluctuation. A value near a threshold on one morning may sit on the other side of it the following week.
It’s suppressed by transient factors. Acute illness, poor sleep, intense recent exercise, significant psychological stress, and substantial calorie restriction all lower testosterone temporarily. A measurement taken during any of these reflects that state rather than a stable baseline.
For these reasons, professional guidelines generally recommend confirming an abnormal result with a second morning measurement before drawing conclusions. A single low reading is a prompt to repeat the test, not a finding.
Total isn’t the same as available
Most circulating testosterone is bound to proteins — largely sex hormone binding globulin (SHBG), with a smaller portion bound to albumin. Only a small fraction circulates unbound.
Testosterone bound to SHBG is generally considered unavailable to tissues. The albumin-bound portion is loosely held and more readily available.
This matters because SHBG levels vary considerably between individuals and are influenced by age, thyroid function, liver function, insulin resistance, obesity, and various medications.
The consequence: two men with identical total testosterone can have quite different amounts of biologically available hormone. A man with high SHBG may have a reassuring total value and low free testosterone. A man with low SHBG — common with insulin resistance and obesity — may have a low-looking total with adequate free hormone.
Measuring total alone can’t distinguish these situations.
What a fuller picture includes
Total testosterone, morning, confirmed on a second occasion.
SHBG, to interpret the total.
Free testosterone, either measured directly by equilibrium dialysis or calculated from total, SHBG, and albumin. Calculated values from standard equations are generally acceptable; some direct assays have known reliability issues.
LH and FSH. These pituitary hormones signal the testes to produce testosterone. Their levels distinguish a testicular problem (low testosterone with elevated LH) from a signaling problem originating higher up (low testosterone with low or inappropriately normal LH). This distinction matters for cause.
Prolactin, where a central cause is suspected.
Symptoms, documented properly. Low testosterone is diagnosed on the combination of consistently low levels and consistent clinical features — not on labs alone.
The reference range problem
Laboratory reference ranges are typically derived from the distribution of values in a sampled population, with the range spanning the middle portion.
Two issues follow. First, ranges are usually not age-stratified in a useful way, though testosterone declines gradually with age. Second, the reference population isn’t necessarily healthy — it reflects whoever was sampled, in a population where obesity and metabolic dysfunction are common, both of which lower testosterone.
Ranges also differ between laboratories and assay methods, making results from different labs not directly comparable.
What often sits underneath
Before treating a low result as a primary problem, it’s worth asking what’s driving it. Several common contributors are addressable:
Obesity, particularly visceral adiposity, which is associated with lower testosterone through several mechanisms. Weight loss is associated with increases in some men.
Sleep apnea, and short sleep generally. Testosterone production is linked to sleep, and restriction reliably lowers it.
Chronic stress.
Substantial calorie restriction or overtraining.
Excess alcohol.
Certain medications, including opioids and corticosteroids among others.
Undiagnosed medical conditions, which is part of why LH and FSH belong in the workup.
The point
Low testosterone is a real clinical entity with real consequences, and men experiencing consistent symptoms deserve proper evaluation rather than dismissal.
Proper evaluation means more than one number. It means morning sampling, confirmation, the binding proteins that determine availability, the pituitary hormones that indicate cause, and a clinician correlating all of it with what you’re actually experiencing.
Anything less is measuring something that fluctuates hourly, once, and treating the result as a fact about you.
